To serve as a Team member for the upcoming Walk, please complete the following  Team Application or download the PDF Team Application and apply by mail.


To sponsor a candidate please complete the following online Sponsor Application or download the PDF Sponsor Application and apply by mail.

Once you have completed the Sponsor Application it is required that your candidate complete the Candidate Application.

  • If you completed the online Sponsor Application and put in your candidate's email address, your candidate will automatically be notified by email to complete the Candidate Application.
  • If you did not input your candidate's email address in the online application, or if you are submitting the PDF Sponsor Application, you will need to download the Candidate Application and have your candidate complete the form. After all applications are complete, please send them to the P.O. Box address below.
  • If you have not paid the deposit online, include a deposit check with the form(s).

NOTE:  The candidate application packet is reviewed by the Board when all components (Sponsor Application, Candidate Application and $50 deposit) have been received.
